Community Living Supports
Offered in numerous neighbourhoods throughout Thunder Bay, these supports assist individuals to live on their own. Supports provided are determined by choice, skills, situation and the desires and needs of the person being supported. This can range from 24-hour support to periodic or monthly visits.
Supports and Services are tailored to individual needs, goals and aspirations as identified in the individualized plan. The plan is developed by facilitating a process of Person Centred Planning. This process involves the individual, family members and other people important to the individual.
Supported Living (Family Home)
This service utilizes private homes within the community in which families or individuals are willing to share their time, their skills and their homes with a person who has a developmental disability. The individual is supported with daily living skills and is included in family activities. Support workers provide assistance with recreational activities, medical appointments, planning meetings etc and ensure that the individual is comfortable and adequately supported in the home.
